MEDICATIONS:
Parent/guardian authorization must be given for ALL medications given at school. ALL medications MUST be brought to the clinic in its original container by an adult. NO child is allowed to carry medications (only exceptions are inhalers and epi-pens, please see the school nurse so that the proper paper work is completed)
Over the Counter Medications:
This includes medications such as Tylenol, Advil, Benadryl, ot Tums. If you feel that your child may need an over the counter medication during the course of the school year, please complete the, "Over the Counter Medication/Non-prescription Medication Form."
Prescription Medications:
If your child requires a prescription medication anytime during the school year. the" Prescription Medication Form" must be completed and will need the prescriber's (physician) signature. The medication must be brought to the clinic in its original container by an adult.
Inhalers and Epi-Pens:
Students that may need to Self-Administer an inhaler or epi-pen at school must complete the appropriate forms for possession and use. The "Authorization for Student Possession and Use of an Asthma Inhaler and/or Authorization for Student Possessions and Use of an Ephinephrine Autoinjector" forms must be signed by both the parent and the prescriber (physician). Please call the clinic with any questions.

Students are not permitted to carry medications or administer medications to themselves without parent/physician written approval; those students caught in violation of this policy are subject to discipline.
